Vital Pulp Therapy is a conservative dental procedure used to treat decay or trauma in young permanent teeth, where the pulp (the innermost part of the tooth) is still alive. Unlike root canal treatment, this therapy focuses on saving the healthy portion of the pulp, allowing the tooth to continue developing naturally.


This is especially important in children and teenagers whose permanent teeth may still be forming.


When Is It Needed?


Vital Pulp Therapy is usually recommended in the following scenarios:

  • A deep cavity that reaches the pulp but doesn’t kill it
  • A traumatic injury that exposes the pulp
  • A tooth with lingering pain to cold or sweets, but no severe infection
  • Teeth that are still developing need a healthy pulp to grow properly.


Types Of Vital Pulp Therapy

  • Direct Pulp Capping: When a small portion of the pulp is exposed, this technique covers the area to promote healing.
  • Pulpotomy: The most common method — removing the infected part of the pulp and leaving the healthy portion to keep the tooth alive and growing.


What To Expect After Vital Pulp Therapy


It's normal to experience mild discomfort or sensitivity after the procedure. This may include:

  • Slight tenderness when chewing
  • Mild gum irritation around the treated tooth
  • Sensitivity to hot or cold foods (usually temporary)
  • Occasional fatigue or fussiness in younger children due to the unfamiliar dental experience


These symptoms are typically short-lived and can be managed with simple care at home.


Immediate Care: First 24–48 Hours


  • Avoid Eating Until Numbness Wears Off: Wait until the local anaesthetic completely wears off before allowing your child to eat. This helps prevent accidental bites to the lips, cheeks, or tongue.
  • Soft Diet Is Key: Offer soft, easy-to-chew foods like mashed potatoes, scrambled eggs, yoghurt, pasta, rice, and smoothies for the first 1–2 days. Avoid anything crunchy, chewy, or hard that could disturb the treated tooth.
  • Use Cold Compresses if Needed: If your child feels sore or swollen, apply a cold compress on the outside of the cheek for 10–15 minutes at a time. This helps reduce any post-treatment inflammation.


Pain Management


  • Over-the-counter pain relief like paracetamol or ibuprofen (as recommended by your endodontist) can help manage discomfort.
  • Avoid aspirin in young children.
  • If antibiotics or anti-inflammatory medication were prescribed, complete the full course as instructed.


Oral Hygiene And Protection


  • Gentle Brushing: Your child can continue brushing and flossing, but they should be gentle around the treated area. Use a soft-bristled toothbrush and ensure they don’t apply too much pressure.
  • Avoid Rinsing Vigorously: Skip mouthwashes or vigorous rinsing for at least 24 hours after treatment. A saltwater rinse (¼ teaspoon salt in a cup of warm water) can be gently swished after meals, if approved by your dentist.


Diet Do’s And Don’ts (After The First 48 Hours)


  • Continue offering nutritious, tooth-friendly foods
  • Avoid sticky sweets, chewing gum, hard candies, popcorn, or ice
  • Limit sugary drinks like sodas and sports drinks, which may irritate the area


Follow-Up Care Is Essential


After Vital Pulp Therapy, it’s important to attend your child’s scheduled follow-up appointment at The Endodontic Office. The dentist will check:

  • How well the pulp is healing
  • If the tooth is continuing to develop normally
  • Whether additional treatment (such as a permanent restoration or crown) is needed


If your child received a temporary filling, it may need to be replaced with a more durable restoration after a few weeks.
